Before Your First Session
Three things to do

Book your appointment
New clients are recommended to begin with the Comprehensive Intake & Assessment Session, $160, 45 minutes. Once confirmed, that time is reserved exclusively for you.

Complete your forms
Every new client completes a Client Liability Waiver & Informed Consent and a Health & Treatment Intake Questionnaire before their first appointment. Both must be done before treatment can begin.

Prepare your space
ORT is a mobile practice. Your session happens where you are. Have a clear, comfortable area ready and send any access details ahead of time.

Why these two forms exist
Manual therapy involves physical contact, pressure, and soft-tissue work. Knowing your history, medications, sensitivities, and preferences is what makes it possible to treat you safely, and to modify or decline a technique when it isn’t appropriate for you.
The questionnaire is not a medical diagnosis and does not replace evaluation by a healthcare professional. If you are unsure whether a treatment is appropriate for you, consult your healthcare provider before your session.
Client Liability Waiver, Informed Consent & Release of Liability
Covers the nature of the services, informed consent, potential risks and temporary effects, products and topicals used, specialized techniques, mobile-service conditions at your location, and the fact that no specific result is guaranteed.
Client Health & Treatment Intake Questionnaire
Covers your goals and current discomfort, medical history, medications (including blood thinners), recent surgeries and cosmetic procedures, pregnancy or postpartum status, allergies and skin conditions, recent illness, injuries, and your pressure and technique preferences.
Cancellation, Rescheduling & No-Shows
The 24-hour policy, in plain terms
ORT Manual Therapy is a mobile, appointment-based service. When you book, that time is reserved exclusively for you, and travel, parking, equipment, and preparation are arranged for your appointment specifically. Here’s the short version.
Notice required for any change
Cancellations and appointment changes must be made at least 24 hours before your scheduled time, measured from the exact appointment time. A 3:00 PM Friday appointment must be changed by 3:00 PM Thursday.
Late cancellation fee
Cancelling with less than 24 hours’ notice is a late cancellation, charged at 100% of the scheduled service price to the payment method on file.
No-shows
Charged at 100% of the service price. A no-show includes being unavailable at your appointment time, being unreachable on arrival, not providing access to the treatment location, or not responding to reasonable attempts to reach you.
Once I’ve left for your location
After departure, the appointment is in the mobile-service travel period. Any cancellation, change, or reschedule request made after that point may be charged at 100%, whether or not I have arrived. Travel time, fuel, and parking can’t be recovered once the trip has begun.
Late arrivals
If you’re not ready at the scheduled time, treatment time may be shortened. Appointments are not automatically extended, and the full price remains due. Delays beyond 15 minutes may be treated as a no-show.
Rescheduling with notice
Reschedule at no charge with at least 24 hours’ notice. Less than 24 hours may be subject to the 100% fee. Repeated cancellations or reschedules may require a deposit or prepayment before another appointment is confirmed.
Genuine emergencies
Unexpected circumstances happen. Exceptions may be granted at ORT’s sole discretion and are not guaranteed. An exception for one appointment doesn’t create an obligation for future ones.
Deposits & prepayment
A deposit or full prepayment may be required to confirm certain appointments. Deposits are applied toward the scheduled service; for a late cancellation or no-show, the deposit may be retained as the cancellation fee to the extent permitted by law.
Mobile Service
Preparing your space
Your session takes place at the location you choose. Clients are responsible for providing a reasonably safe, clean, accessible, and appropriate environment for treatment, and for letting ORT know in advance about anything that could affect access or safety.
If a location can’t reasonably be accessed because of incorrect information, lack of entry, unavailable parking, or building restrictions, the appointment may be treated as a no-show.
Please have ready
- The correct service address
- Safe, reasonable access to the location
- Building, gate, parking or entry instructions
- Any required access codes or permissions
- Adequate, clear space for the treatment setup
- Advance notice of pets, stairs, or access limitations
Please also secure valuables, jewelry, and electronics before your session. ORT is not responsible for personal property except where liability cannot legally be waived.
