Call or email with the session you want, your preferred day and time, and the address where you’d like to be treated.

We confirm
I confirm the time, the location, and anything I need to know about access, parking, or your space. Your appointment time is then reserved exclusively for you.

Forms before we meet
New clients complete a liability waiver and a health intake questionnaire before the first appointment. How that works →

Everything needed for the session arrives with me. Have a clear, comfortable space ready.
New Clients
Your waiver & health intake
Every new client completes two documents before their first session: a Client Liability Waiver & Informed Consent and a Health & Treatment Intake Questionnaire.
Both are provided to you once your appointment is confirmed, and can be completed in advance or in person at the start of your first session.
Email is not a secure way to send medical information. Request the forms by email if you like, but complete the health questions on the form itself, not in the body of an email or a text message.
A 24-hour cancellation, rescheduling and no-show policy applies to every appointment. Booking confirms that you have read and agree to it.
